Shelter Street Outreach Coordinator

 

Stephen Center has served homeless and low-income individuals in Omaha since 1984 with the mission to partner with the community, families and individuals to overcome homelessness, addiction and poverty.  The Pettigrew Emergency Shelter and Permanent Supportive Housing offerings provide safe and secure housing while individuals and families complete an assessment of permanent housing options. All Stephen Center residents have access to dual diagnosis support services via the HERO addiction/recovery program. 100% of Stephen Center shelter supportive housing participants are living in poverty.  Housing transition support identifies poverty barriers that may impact one’s ability to achieve or maintain permanent housing through case management, life skills, and behavioral health services. Stephen Center serves approximately 250 people daily in all programs.

 

Purpose:

The Street Outreach Coordinator plays a pivotal role in Stephen Center’s mission to provide support, resources, and pathways to stability for individuals experiencing homelessness. This position combines field-based outreach with program coordination, ensuring that unsheltered individuals, shelter residents, and community partners experience consistent, compassionate, and effective services.

In addition to direct outreach and case management, the Street Outreach Coordinator is responsible for fostering community partnerships, coordinating across Continuum of Care (CoC) teams, and contributing to the creation and implementation of policies and procedures that ensure safe, trauma-informed, and effective service delivery in unpredictable environments.

Key Accountabilities:

Street Outreach & Case Management

  • Conduct proactive outreach to individuals and families in encampments, public spaces, and other unsheltered areas in Douglas, Sarpy, and Pottawattamie counties.
  • Build trusting relationships with clients, assess immediate needs, and connect them to shelter, housing, healthcare, and other resources.
  • Provide crisis intervention and de-escalation while ensuring personal and client safety.
  • Develop individualized case management plans for unsheltered individuals and shelter residents, including goal setting and progress monitoring.
  • Maintain accurate, timely documentation and enter service data into Clarity within 24 hours.

Program Coordination & Oversight

  • Act as a key liaison with Threshold Continuum of Care (CoC) and other outreach providers to ensure coordinated services.
  • Support Stephen Center shelter operations by bridging information between outreach efforts and shelter admissions.
  • Assist with shelter client case management when needed, ensuring continuity of care between unsheltered and sheltered populations.
  • Ensure all policies, procedures, and compliance standards (including CARF and funder requirements) are followed.

Community Partnerships & Representation

  • Develop and strengthen relationships with community partners, law enforcement, healthcare providers, and housing agencies.
  • Represent Stephen Center at community meetings, collaborative task forces, and training sessions.
  • Advocate for trauma-informed, humane approaches to homelessness across all partnerships.

Policy, Procedure & Safety

  • Collaborate with leadership to create, implement, and refine policies and procedures for outreach and shelter coordination.
  • Ensure consistency and clarity in safety protocols for staff and clients in unpredictable environments.
  • Provide training, guidance, and feedback to team members to strengthen field safety, professional boundaries, and effective client engagement.

Qualifications

  • Bachelor’s degree in social work, Psychology, Human Services, or related field (equivalent experience considered).
  • At least 1 year of supervisory or program coordination experience preferred.
  • Demonstrated experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders.
  • Strong interpersonal and crisis management skills, with the ability to build rapport across diverse populations.
  • Knowledge of Omaha-area community resources strongly preferred.
  • Valid driver’s license and reliable transportation required.

Working Conditions

  • Combination of office, shelter, and field-based work in varying environments, including outdoor encampments and public spaces.
  • May involve exposure to challenging or unpredictable situations, requiring safety awareness and trauma-informed practice.
  • Evening or weekend hours will be required.
  • Physical requirements: ability to sit, stand, walk, kneel, and lift up to 20 lbs.
